PIAA Physical Form - Sections 6-7
Section 6 - Must be completed by parent/guardian Section 7 - Must be completed and signed by Physician Physicals for the 2021-22 School year must be dated after June 1, 2021
PIAA Recertification Form
Section 8 - Only needed for subsequent sports within the same school year (example - student plays a fall and winter sport - this form is needed for the winter sport). Section 8 can be completed on Family ID as well.
